Medical Termination of Pregnancy: What You Should Know About It

Making the decision to end a pregnancy is deeply personal and often very difficult. If you are reading this, you may be facing an unplanned pregnancy and considering your options. You are not alone, and you do not need to face this decision in isolation or fear.

As a gynecologist with more than 30 years of experience providing compassionate, non-judgmental care, my role is to give you accurate information, safe medical care, and emotional support throughout this process.

Our clinic is government approved to provide Medical Termination of Pregnancy (MTP) up to 12 weeks of gestation. All procedures are legal, safe, and completely confidential.

Understanding MTP in India

Legal status
Medical Termination of Pregnancy is legal in India under the MTP Act of 1971, which was amended in 2021. The law allows:

  1. MTP up to 20 weeks with the opinion of one registered medical practitioner.
  2. MTP up to 24 weeks in special circumstances such as rape, fetal abnormalities, and certain vulnerable groups.
  3. Our clinic is specifically approved to provide MTP up to 12 weeks of pregnancy.

Your rights

  1. Adult women (18 years and above) do not need consent from a partner or family member.
  2. Your confidentiality is protected by law.
  3. You have the right to seek safe, legal MTP services.
  4. You have the right to be treated with dignity, respect, and without judgment.

Important facts

  1. MTP is not the same as illegal MTP.
  2. When performed in an approved facility by a qualified gynecologist, MTP is very safe.
  3. Complications are rare when proper protocols are followed.
  4. MTP does not generally affect future fertility when done safely.

Who Chooses MTP

Women from all backgrounds may seek MTP, including:

  1. Young women who are not ready for motherhood.
  2. Women who have completed their families.
  3. Women with health issues that make pregnancy risky.
  4. Women facing difficult personal, social, or financial circumstances.
  5. Victims of rape or incest.
  6. Women carrying fetuses with significant abnormalities.
  7. Women whose contraception has failed.

Whatever your reason, it is personal and valid. You deserve respectful and compassionate care.

Your Options

Broadly, you have three choices when faced with an unplanned pregnancy:

  1. Continue the pregnancy and parent
    You may choose to raise the child yourself, with or without support from your partner or family.
  2. Continue the pregnancy and choose adoption
    You may decide to give birth and place the baby for adoption. Different adoption arrangements can be discussed.
  3. Medical Termination of Pregnancy
    You may decide to medically end the pregnancy. There are two safe methods available, depending on how far along the pregnancy is.

Only you can make this decision. My role is to provide medical guidance, not pressure.

Two Safe MTP Methods

Method 1: MTP by tablet (medical method)

Suitable for
Pregnancies up to 9 weeks (63 days from the first day of the last menstrual period).

How it works
This is a non-surgical method using two medications taken at different times.

Step 1: Mifepristone
Taken in the clinic under supervision.
It blocks progesterone, the hormone needed to maintain pregnancy.
Most women do not feel major changes immediately after this tablet.

Step 2: Misoprostol
Taken at home after 24 to 48 hours, as per instructions.
It causes the uterus to contract and expel the pregnancy tissue.
Cramping and bleeding usually start within a few hours.

What to expect

  1. Cramping similar to strong menstrual cramps, sometimes more intense.
  2. Bleeding heavier than a normal period, with clots.
  3. Most women pass the pregnancy within 4 to 6 hours after the second medicine.
  4. The process is usually complete within 24 hours but in some cases may take a few days.

Advantages

  1. Non-surgical method.
  2. More privacy, as most of the process happens at home.
  3. No anesthesia needed.
  4. Less invasive than a procedure.

Disadvantages

  1. Takes several days to complete fully.
  2. Bleeding and cramping are usually more than with suction MTP.
  3. You are more aware of the process as it happens.
  4. Slightly lower success rate compared to suction (about 95 to 98 percent).
  5. Follow-up is required to confirm that the pregnancy has been completely expelled.

Success rate
Approximately 95 to 98 percent.

Method 2: MTP by pain-free suction (surgical method)

Suitable for
Pregnancies up to 12 weeks.

How it works
This is a quick procedure done in the clinic.

  1. You will receive sedation or local anesthesia so that you do not feel pain.
  2. Gentle suction is used to remove the pregnancy tissue from the uterus.
  3. The actual procedure usually takes about 10 to 15 minutes.
  4. You rest in the clinic for 1 to 2 hours afterward.
  5. You go home the same day.

What to expect
During the procedure
You may feel some pressure or stretching, but with proper anesthesia you should not feel pain.

After the procedure
Mild to moderate cramping and bleeding.
Bleeding is often similar to or lighter than a period and may last 1 to 2 weeks.
Most women feel physically normal within 1 to 2 days.

Advantages

  1. Very quick procedure.
  2. Pain-free during the procedure due to anesthesia.
  3. Immediate completion of the process.
  4. Very high success rate (more than 99 percent).
  5. Less bleeding and cramping compared to the tablet method.
  6. You do not see the process happening in the same way as with tablets.
  7. Can be used later in early pregnancy, up to 12 weeks.

Disadvantages

  1. Requires you to visit the clinic for the procedure.
  2. Needs anesthesia or sedation.
  3. Slightly more invasive than the tablet method.
  4. Very small risk of uterine perforation or injury, which is rare in experienced hands.

Success rate
More than 99 percent.

Which Method Is Right for You

Choose MTP by tablet if:

  1. Your pregnancy is less than 9 weeks.
  2. You prefer a more natural, stepwise process.
  3. You want more privacy at home.
  4. You are prepared for heavier bleeding and cramping.
  5. You wish to avoid a procedure with instruments.

Choose MTP by suction if:

  1. Your pregnancy is between 9 and 12 weeks (in this period, suction is usually the only option).
  2. You want a fast, same-day completion.
  3. You prefer less bleeding and cramping overall.
  4. You want a pain-free experience during the procedure.
  5. You prefer not to witness the process in detail.
  6. You want the highest success rate.

Both methods are safe and effective when done correctly. We will help you decide based on how many weeks pregnant you are, your medical history, and your personal preferences.

The Process at Our Clinic

Step 1: Initial confidential consultation

This visit includes:

  1. Pregnancy confirmation.
  2. Ultrasound to determine how far along the pregnancy is.
  3. Detailed medical history review.
  4. Blood tests such as hemoglobin and Rh factor.
  5. Explanation of both methods (tablet and suction).
  6. Time to ask all your questions.
  7. Consent process and documentation.
  8. No pressure to decide immediately.

Step 2: The procedure

For MTP by tablet

  1. The first medicine is given in the clinic.
  2. You will receive clear written instructions about when and how to take the second medicine at home.
  3. Pain relief medicines are prescribed.
  4. Our emergency contact number is given to you.
  5. A follow-up appointment date is fixed.

For MTP by suction

  1. You come to the clinic on the scheduled day.
  2. You may need to fast for about 6 hours beforehand.
  3. You should bring an adult companion to take you home.
  4. You change into a hospital gown.
  5. An intravenous line is placed.
  6. Sedation or anesthesia is given.
  7. The procedure is completed in about 10 to 15 minutes.
  8. You rest in the recovery area for 1 to 2 hours.
  9. You are discharged with medicines and after-care instructions.

Before-care instructions

For MTP by tablet

  1. Keep enough sanitary pads at home.
  2. Keep pain medicines such as ibuprofen as advised.
  3. Plan to stay at home and rest for 2 to 3 days.
  4. Try to have someone around for support if possible.
  5. Keep our clinic’s emergency number accessible.
  6. Arrange for light, simple food and adequate fluids.

For MTP by suction

  1. Do not eat or drink anything for 6 hours before the procedure, if advised.
  2. Arrange transport and a companion to take you home afterward.
  3. Wear comfortable, loose clothing.
  4. Remove jewellery and contact lenses.
  5. Empty your bladder before the procedure.
  6. Bring sanitary pads.
  7. Plan to rest at home for at least 24 hours.

After-care instructions

First 24 hours

  1. Rest as much as possible.
  2. Expect bleeding; this is normal.
  3. Take pain medicines exactly as prescribed.
  4. Use a hot water bottle or heating pad for cramps if needed.
  5. Eat light meals and drink plenty of fluids.
  6. Do not drive if you have had sedation or anesthesia.

First week

  1. Gradually increase your activity as you feel better.
  2. Avoid heavy lifting and strenuous exercise.
  3. Avoid sexual intercourse.
  4. Do not use tampons; use only sanitary pads.
  5. Do not douche.
  6. Avoid swimming or taking a bath; showers are allowed.
  7. Take all prescribed medicines, including antibiotics if given.
  8. Observe your bleeding pattern; it should gradually reduce.

What is normal

  1. Bleeding like a heavy period, possibly with clots.
  2. Cramping that improves with pain medicine.
  3. Light spotting for up to two weeks.
  4. Breast tenderness.
  5. Feeling tired.
  6. Emotional ups and downs.

Warning signs: when to call immediately

Contact us or seek urgent care if you experience:

  1. Very heavy bleeding, such as soaking through two or more large pads per hour for more than two consecutive hours.
  2. Severe pain that does not improve with prescribed pain medicines.
  3. Fever higher than 100.4°F (38°C).
  4. Foul-smelling vaginal discharge.
  5. Severe weakness, dizziness, or fainting.
  6. No bleeding at all after taking MTP tablets, which is unusual and needs evaluation.
  7. Signs of an allergic reaction such as rash, swelling, or difficulty breathing.

Recovery Timeline

Physical recovery

  1. Days 1 to 2: Rest, bleeding, and cramping.
  2. Days 3 to 5: Energy gradually returns and bleeding reduces.
  3. By week 2: Most physical symptoms are resolved.
  4. By week 4: Complete physical recovery in most women.
  5. Your next period usually returns in 4 to 6 weeks.

Emotional recovery
Every woman processes the experience differently. Some feel relief, some feel sadness or grief, and many feel a mixture of emotions. All these responses are normal.

When you can

  1. Return to work or college in about 2 to 3 days, or whenever you feel ready.
  2. Resume light exercise after about one week.
  3. Resume sexual intercourse after a minimum of two weeks and after the follow-up visit confirms that you are healing well.
  4. Become pregnant again very soon after MTP if you do not use contraception.

Step 3: Follow-up visit (around 2 weeks later)

This visit is important and should not be skipped. It includes:

  1. Ultrasound to confirm that the uterus is empty and the process is complete.
  2. Physical examination.
  3. Check on your recovery and address any symptoms.
  4. Discussion and initiation of contraception to prevent another unplanned pregnancy.
  5. Space to talk about any emotional concerns.

Contraception After MTP

You can become pregnant again even before your next period, so contraception is important.

Typical start times

  1. Pills: Often started on the day of the procedure or the next day.
  2. Injection: Can be given on the same day.
  3. IUD: Can sometimes be inserted at the time of suction MTP or after the follow-up visit.
  4. Implant: Can often be inserted at the time of the procedure.
  5. Condoms: Can be used as soon as you resume sexual intercourse.

Safety and Complications

How safe is MTP
When done in a government-approved center by a qualified gynecologist:

  1. MTP is very safe.
  2. Serious complications are rare (less than 1 percent).
  3. It is much safer than undergoing an unsafe, illegal MTP.
  4. It is also medically safer than continuing a pregnancy in many high-risk situations.

Possible complications (rare)

  1. Incomplete MTP, where some tissue remains: more common with tablets (2 to 5 percent) and rare with suction.
  2. Infection: risk is low and usually prevented with antibiotics.
  3. Heavy bleeding requiring further treatment: uncommon.
  4. Allergic reaction to medicines: rare.
  5. Uterine perforation with suction method: extremely rare in experienced hands.

Most complications are treatable, especially when recognized early.

Future fertility
Safe MTP in an approved facility does not usually cause infertility. Most women can conceive again and have healthy pregnancies when they are ready.

Emotional Aspects

Common feelings after MTP may include:

  1. Relief.
  2. Sadness or grief.
  3. Guilt.
  4. Anger.
  5. A feeling of emptiness.
  6. Peace with the decision.
  7. A mixture of several emotions at once.

All of these are normal and valid.

Coping strategies

  1. Allow yourself to feel your emotions without judgment.
  2. Talk to a trusted person or counsellor.
  3. Write down your thoughts and feelings.
  4. Take care of your physical health with rest, food, and fluids.
  5. Give yourself time to heal emotionally.
  6. Seek professional help if your feelings are overwhelming or persistent.

Consider seeking extra support if you notice:

  1. Ongoing sadness or depression.
  2. Difficulty functioning in daily life.
  3. Intense guilt or regret that does not reduce with time.
  4. Strain in relationships.
  5. Persistent sleep or appetite problems.

Confidentiality and Privacy

Common Questions

Is it really legal
Yes. MTP is legal in India, and our clinic is government approved for this purpose.

Will you inform my parents or husband
No. If you are 18 years or older, we are not allowed to disclose your information without your permission.

What if I change my mind
You can change your mind at any time before starting the actual procedure. There is no pressure from our side.

How will I know which method is better for me
We will assess how many weeks pregnant you are, your medical history, and your preferences, then discuss both methods clearly. The final decision rests with you.

Will it be painful
Tablet-based MTP causes cramping similar to strong period pain, which usually responds to pain medicines. Suction MTP is done under anesthesia, so you should not feel pain during the procedure, though mild cramps afterwards are expected.

When can I have sex again
Generally after at least two weeks and after your follow-up visit confirms that you have healed. You should use contraception if you do not want another pregnancy.

Will people be able to tell that I had an MTP
No. There are no external signs. This remains private unless you choose to share it.

Will I be able to have children later
Yes. Safe MTP does not usually affect future fertility. Many women have safe, healthy pregnancies later.
